As your Home institution, the Office of Admissions and Student Affairs will provide the following during the verification of student credentials process in the VSLO application service.
- Official UNMC transcripts (a transcript request is not required)
- This student is in good academic standing at this institution.
- This student has been instructed in OSHA safety measures and infection control precautions.
- This student has a current ACLS. (Yes, after June/July)
- This student has a current BLS.
- You can download a copy of your BLS card through the American Heart Association.
- If you haven't picked up your original card you may have to receive an access point from emstrauma@unmc.edu.
- You can download a copy of your BLS card through the American Heart Association.
- This student has completed a Mask Fit Test. (Issuing agency has record if needed)
- This student is taking electives for credit.
- This student will pay tuition at the home institution (UNMC) during the rotation.
- Medical liability and/or malpractice insurance will be covered by the home institution (UNMC) during the rotation. (at $3M aggregate and $1M per instance)
- Certificate available upon request to vslo@unmc.edu.
- We require our student to hold personal health insurance.
- This student will have successfully completed these core clerkships: Family Medicine, Internal Medicine, Obstetrics & Gynecology, Pediatrics, Psychiatry, Surgery.
- This student will be in their senior year at the time of the elective(s).
- This student is expected to graduate in __________.
- This student has met all immunization requirements or student health requirements as defined by our school. (link provided to our requirements)
- This student has complied with HIPAA training requirements.
- You can download an official certificate from CARE.
- This student has completed a criminal background check at our institution.
- Copy of the background check available upon request to vslo@unmc.edu.
- If the Host institution requires a more recent background check, a rescreen can be initiated through One Source.
- This student has passed USMLE Step 1.
